 Our expertise is in the science of Gerontology. Gerontology is defined as the scientific study of the biological, psychological, and sociological effects associated with old age and aging. This also encompasses the branch of medical science that deals with diseases and problems specific to seniors. We not only have worked in the field with seniors, but have spent our time studying about them. We have hands on experience in elder care situation and have devoted our time to studying the population and learning as much as possible about the elder community and its members in the collegiate world. 

 

There are many people out there that say that they are educated in the field of geriatrics but are not educated specifically in the senior population. We are different, we studied this population solely and have the answers you need to help end your confusion when navigating  the elder care system. We offer up to date information and are able to offer the most current knowledge that is available, to our clients.

 

We also have an extensive amount of knowledge about the Special Needs community and have completed programs in Special Education and how it relates to this population. Many times society has forgotten about this population and how important it is to discuss their options when they age. We are here to help navigate the system for clients who are developmentally disabled or delayed, and have just as much knowledge in this population as well as in the "typical" aging population. 

 

With this knowledge, Linbrooke Geriatric Care Associates is able to offer assessments that search for information on a "whole" client approach. We assess all aspects of the life of the client. We look down every avenue and provide all possible options, even ones that you may have never thought that you may have had. We are looking at the client as a whole, and even if something seems like it won't affect the client, it just might. We are here to offer the client the brightest, most independent future, even if they didn't think that they had certain options, we make options possible.

 

When we administer our assessments to our clients we  assess many different aspects of life. We look at the functionality, emotional, financial and physical capabilities of our clients, and sometimes the caregivers/family that is most present in their life and care as well. 

 

We assess these aspects of the client's life in order to determine the present state of the client as well as use this information to determine what their most independent future could look like.  We administer these assessments in order extract the information needed to answer the questions or provide ways around the obstacles that are presently affecting the life of the client and their family. These care plans offer the most personalized, detailed, and easy to follow steps to help the client to live the most healthy, independent life possible while keeping their dignity and respect with an understanding that though they may be faced with an obstacle it can be worked with or through with well educated and knowledgeable recommendations that are provided in the care plan.

 

   As  Geriatric Care Managers, we administer  thorough assessments, arrange care services, find community resources, assist with residential placement, and become your source for the most up to date information. We are who you need to make the best decisions about your loved ones future, as well as being there to help your elder with any other questions they may have.  We are available 24 hours a day, 365 days a year to help with anything that you or your loved one needs as well as anything that you need to make you or your elder loved ones life more comfortable and independent. 

Our Business

After the initial phone inquiry where we determine the obstacles being faced and the questions that need to be answered, we meet with new clients and family members in the client's residence. This is where we assess the client's medical, cognitive and social needs, as well as the current living situation of the client.  If interviews of other people must be made, they are done after the initial assessments are administered.  After we gather all the information that is deemed necessary in order to draw up the most informed care plan, we prepare and present a written care plan with professional and personalized recommendations that relate to physical, emotional, and social needs of our client.  These recommendations are then discussed with the client and the client can either choose which ones to utilize or decide that they would like more information about other options that were not looked into. 

 

If the client chooses recommendations within the present care plan, they then can choose to have Linbrooke Geriatric Care Associates implement the recommendations chosen. This is where Linbrooke Geriatric Care Associates work for the client to ensure that every step is made to overcome or work through  the obstacles that they came to us with and their life is set up to work for them with the recommendations at the center of it. Whether it be a residential placement, home care agency, or entitlements, we will set up and put in place all of the necessary paper work, and leg work that is needed to ensure a healthy and bright future for you or your loved one.  Linbrooke Geriatric Care Associates specializes in implementing care plans and reaching goals for our clients.

My active participation in these exclusive associations,provide me with new, and exciting knowledge on a daily basis to help in making informed recommendations that I provide in care plans to my clients. I am constantly updating and making necessary improvements to the assessments that are used in order to extract the necessary information from my clients to draw up these care plans and am involved in many professional development opportunities in order to ensure that I am at the peak of my knowledge and education of the ways to navigate the elder care system.

 

I am presently involved with the Special Needs population and understand the inner working of the social service system. I not only feel that I am always improving my skills and educating myself in the Gerontological field but by doing so am able to provide my clients with the most informed options and recommendations that give my clients the ability to have a choice. It is choices that give my clients the ability to remain independent and still able to live their life's the way that they want to with only slight adjustments for what life has handed them. I provide insight, and knowledge in order to aid you and your loved ones in making great choices for you. My clients are like my family, and that is how I treat you.
