History

   I felt the need to  start Linbrooke Geriatric Care Associates after watching my family's
questions go unanswered by doctors and caregivers.  As my grandmothers health declined, I
decided the direction that I wanted to take was to help, aid, and service the elderly
population.  It dawned upon me, that I wanted to provide information, insight, and help, to
people that do not know where to start looking for answers but know that they want to
ensure the happiness, and comfort of themselves or their loved ones.   

  Linbrooke Geriatric Care Associates was formed to ensure that ourclients receive the most
personalized assessments, detailed care plans, as well as individual personal attention.  In
addition we provide knowledgeable information, guidance, crisis intervention,consultation,
Home Care services, entitlement's, advocacy, placement, and referrals.

What We Do

     As Professional Geriatric Care Managers we evaluate your situation, identify areas that
could be improved upon, provide solutions, and work with you to design a plan for
maximizing  independence and well being.  We also arrange for special services and monitor
your care on a one to one basis.  Each client is a new personal relationship and is provided
unique care plan.  

           
As Geriatric Care Associates we help:

  • those who are new to elder care or uncomfortable with elder care decision-making
  • people who are having difficulty with any aspect of elder care
  • families that are faced with a sudden crisis or a major change in one's lifestyle
  • those who are dealing with a complex situation such as a psychiatric, cognitive, health,
    legal or social issue.


      After the initial phone inquiry, we meet with new clients and family members usually in
the home or client's residence. There we assess the client's medical, cognitive and social
needs, as well as the current living situation.  We then prepare and present a written care
plan with professional recommendations that relate to physical, emotional, and social needs
of our client.  We also take into consideration the financial resources available to meet our
goals.

    The goal of each plan is to enable the elder to continue to live at home as independently as
possible or to successfully transition them into a facility they are most comfortable with.  
This may consist of either a retirement community, a skilled care center, or in home
assistance.  Often, we are asked to implement the approved plan in collaboration with the
family.

   We are also available to link caregivers with respite care, as well as information to
caregivers to relieve stress, anxiety, and offer knowlegable and researched ideas to help with
the caregiving
process.

  I am a member of The Association for Gerontology in Higher Education, The
Gerontological Society of America, the International Association of Gerontology, and The
National Association of Care Planning.  My active participation in these exclusive
associations,provide me with new, and exciting knowledge daily. I am involved with the
disabled population and understand the inner working of the social service system.     I am
provided with insight, and knowledge in order to aid you and your loved one with the best,
and most up to date information.

What does a Geriatric Care Manager do?

   Geriatric Care Managers are specialists in assisting older people and their families to
plan for and implement ways to allow for the greatest degree of independence, safety and
comfort. They meet with families, assess the client's needs, and develop a comprehensive
plan with input from caregivers, professionals, and others. They then assist with any
appointments, interviews, and other paperwork and make all preparations for needed
in-home care or arrange for a transition to a facility. They review all options clearly and
discuss them with the family. Once services are set up, they can monitor and coordinate
them, and revise the plan if needed. This gives the family assurance that an objective,
knowledgeable professional is available to maintain personal contact with the older
